KEY REQUEST FORM
Faculty and Staff may use the Key Request Form below to request keys for yourself or the students that you supervise.
Students may use this page to request keys for themselves (this method will require supervisor approval – once the request is processed, supervisors will receive an email requesting a final approval).
BUILDING ACCESS REQUEST FORM
This form is used by Faculty, Staff, and Students to submit your wiscard information and access requirements so that MSC building access may be added to your wiscard. **Note that this is a UW Google form that can only be accessed while logged in to your UW–Madison Google account.
